SARS-CoV-2 վարակի բուժվարումը հիմնականում պայմանավորված է շնչուղիների ախտահարման բարդությամբ։ Ընդհանուր բնակչության շրջանում Քովիդ-19-ի ընթացքն առանց ախտանշանների է կամ թույլ արտահայտված ախտանշաններով, ինչը պահանջում է ամբուլատոր բուժում` կախված հանգամանքներից 5-10 օր մեկուսացման միջոցառումներ ապահովելով: Ամեն հնարավոր դեպքում թույլ իմունիտետով բուժառուներին հարկավոր է առաջարկել մինչշփումային կանխարգելում միակլոնային հակամարմիններով: Հակացուցումների բացակայության դեպքում նիրմատրելվիր-ռիտոնավիր (Պաքսլովիդ, Paxlovid) համակցության հաբերը ներկայումս Քովիդ-19-ի ամբուլատոր բուժման լավագույն տարբերակն են մեծահասակների համար, որոնք չունեն թթվածնային բուժման կարիք, և չկա ծանր ընթացքի վտանգ։ Կորտիկոստերոիդային և հակամակարդիչ բուժումները թթվածնի կարիք ունեցող հոսպիտալացված հիվանդների վարման հիմնական լրացուցիչ միջոցներն են` համակցված կանխարգելիչ միջոցառումների հետ: Դրանց նպատակը երկրորդային դեպքերը բացառելն է: Հակավիրուսային մոտեցումը սահմանափակ կիրառություն ունի այս համատեքստում, հիմնված է պասիվ իմունաբուժության (հիմնականում չեզոքացնող միակլոնային հակամարմիններ) կամ հակավիրուսային դեղամիջոցների վրա, որոնք ներգործում են գերիշխող շտամի վրա և ցուցաբերել են կլինիկական արդյունավետություն
La prise en charge médicale de l’infection par le SARS-CoV-2 est principalement guidée par la sévérité de l’atteinte respiratoire. Dans la population générale, l’infection est le plus souvent asymptomatique ou paucisymptomatique, autorisant une prise en charge ambulatoire, sous réserve de mesures d’isolement de cinq à dix jours selon les circonstances. La prévention préexposition par anticorps monoclonaux doit être proposée chaque fois que possible chez les patients immunodéprimés. En l’absence de contre-indication, l’association nirmatrelvir-ritonavir (Paxlovid) est actuellement le traitement curatif oral ambulatoire chez les patients adultes ne nécessitant pas d’oxygénothérapie et à risque de forme grave. Pour les patients hospitalisés oxygénorequérants, la corticothérapie et l’anticoagulation sont les principaux traitements complémentaires, en association aux mesures de prévention pour l’éviction des cas secondaires. L’approche antivirale est limitée dans ce contexte, reposant sur l’immunothérapie passive (anticorps monoclonaux neutralisants essentiellement) ou des antiviraux actifs sur le variant dominant et ayant démontré une efficacité clinique.
Management of Covid-19 is primarily guided by the severity of the respiratory involvement. Covid-19 is mostly asymptomatic or pauci-symptomatic allowing the ambulatory setting associated with isolation measures going from 5 to 10 days depending on the circumstances. Nirmatrelvir/ritonavir (Paxlovid) is currently the oral treatment for Covid-19 in adult patients not requiring oxygen therapy and at risk of severe disease in the absence of contraindications. For hospitalized patients requiring oxygen support, corticosteroid therapy and anticoagulation are the main supportive treatments in association with measures preventing secondary cases. The antiviral approach is limited in this context, consisting in passive immunotherapy (mostly neutralizing monoclonal antibodies) or direct-acting antivirals active on the dominant variant and that have demonstrated clinical efficacy.
